Eligibility
Residents of New York State and the New York metropolitan area who are 18 years or older are invited to take part in this research study. The purpose of the survey is to learn about how transportation, employment, disability, residential satisfaction, and use of mobile technology affect quality of life.
Participant Incentive
New York State residents who complete the survey are eligible for a raffle prize. Due to other states’ regulations regarding games of chance, residents of states other than New York are not eligible for the raffle drawing. You can submit your answers anonymously or enter your email address at the end of the survey. New York residents who complete the survey and submit their email address by 11:59 pm EST on February 28, 2020 will be considered for one of twenty (20) $50 raffle prizes, with a drawing on February 29, 2020.

Procedures
If you agree to participate, we will ask you to complete an online survey. It includes questions about
- you and your family;
- any disabilities you have;
- your wellbeing;
- your education and employment experience;
- your satisfaction with your home;
- your transportation behaviors and experiences;
- your use of technology;
Time Commitment
The survey will take 10-15 minutes to complete. If you use the same device (computer or smartphone) with cookies enabled, your answers will be stored for up to one week if you don’t want to complete the survey in one sitting.
Potential Risks or Discomfort
There are few potential risks or discomfort associated with this study. If you have privacy concerns, you can complete this survey without submitting your contact information.
